Write a model card outline for a hospital readmission model

A hospital has built a gradient-boosted model that predicts 30-day readmission risk from electronic health records and plans to use it to prioritise follow-up calls. Write the outline of its model card: which sections, what each must contain, which numbers and slices must be reported, which parts come from a datasheet, and how the card connects to governance.
